Parent/Guardian and Student-Athlete should fill out the Winter Athletic Registration Form that can be found in our google classroom. 

In addition to the Winter Athletic Registration Form, each student-athlete is required to submit the following to the athletic office:

1. Medical History and Emergency Permission Form (please click link to see form)

   1. Now a fillable PDF that can be typed into, saved, and sent as an attachment. 

2. Updated Physical Examination (within 13 months) 

   1. Signed by a Physician

All of these documents must be submitted as a scan or hard copy to Athletic Assistant, Caroline Gattuso (caroline.gattuso@wpsk12.com) or Director of Athletics, Mia Muzio (mia.muzio@wpsk12.com). 

PICTURES OF REQUIRED PAPERWORK WILL NOT BE ACCEPTED.
